UnitedHealthcare IPA HMO
The UnitedHealthcare IPA HMO is available to retired, eligible LACERA members and their dependents who are under age 65 and not eligible for Medicare. An IPA model consists of physicians in private practice that contract with an HMO to provide services to their members.
Features of this plan:
- You must receive all medical care through UnitedHealthcare providers
- No deductibles
- No claim forms
- Copayments are required for certain services
UnitedHealthcare Group MAPD HMO
UnitedHealthcare Group MAPD HMO is available to retired, eligible LACERA members and their dependents who are age 65 or older and enrolled in Medicare Part A and B.
Combined coverage: If you have Medicare Parts A and B and enroll in the UnitedHealthcare Group MAPD plan, your non-Medicare eligible family members must enroll in the UnitedHealthcare IPA HMO plan. See more about combined coverage on the Parts A and B page.

Solutions for Caregivers
This care management service is designed to serve caregivers who are providing critical support to Medicare Advantage Group Retiree members. The care management service is delivered by experienced professionals with expertise in geriatric care and caregiver support.
Service is rooted in understanding caregiver and care recipient issues, and providing resources to help meet caregiving needs.
Benefit features:
- Telephonic care management (six hours/year), or
- Personalized caregiver care plan (as needed/appropriate)
- Upon request, an on-site assessment conducted by a nurse to evaluate the care recipient’s needs, with recommendations from the care manager on the best use of the benefit
- Monthly caregiver education sessions
